Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum of 2 years, with acute hospital and travel experience required License: Must hold a valid state license Certifications: BLS required Must-Have: Acute hospital experience, ability to float to medical units, ICU/CCU, and maternal child Description: The Case Manager RN is responsible for coordinating patient care across different departments. This role requires strong skills in care coordination and discharge planning. Acute hospital experience is essential, as is familiarity with utilization criteria. The Case Manager will float among medical units and other areas as needed. Weekend coverage is expected on an every-other-weekend basis. The successful candidate will have experience working with adult and geriatric populations. Supporting services include physical therapy, social services, and pharmacy among others. This position involves reviewing patient admissions and continued stay requirements. Two supervisor references are required. Candidates must complete specific training modules upon hire.
